Estimate VLDL-C and remnant cholesterol from a fasting triglyceride level.
Very Low Density Lipoprotein (VLDL) is the primary triglyceride-rich particle produced by the liver. As VLDL circulates, lipoprotein lipase strips triglycerides, leaving smaller, denser remnant particles that ultimately become LDL. VLDL itself is atherogenic and contributes to cardiovascular disease independently.
The TG/5 estimation is unreliable when triglycerides exceed 400 mg/dL or in non-fasting samples. Newer methods (e.g., Martin/Hopkins, Sampson equation) provide better VLDL estimation across the lipid spectrum.
