VLDL Cholesterol Calculator

Estimate VLDL-C and remnant cholesterol from a fasting triglyceride level.

VLDL-C ≈ TG ÷ 5 (mg/dL) — valid when TG < 400 mg/dL and patient is fasting.

What Is VLDL?

Very Low Density Lipoprotein (VLDL) is the primary triglyceride-rich particle produced by the liver. As VLDL circulates, lipoprotein lipase strips triglycerides, leaving smaller, denser remnant particles that ultimately become LDL. VLDL itself is atherogenic and contributes to cardiovascular disease independently.

Normal Range & Interpretation

  • Normal: 2–30 mg/dL (0.05–0.78 mmol/L)
  • Borderline high: 30–40 mg/dL
  • High: >40 mg/dL — increases CVD risk

Limitations of TG/5

The TG/5 estimation is unreliable when triglycerides exceed 400 mg/dL or in non-fasting samples. Newer methods (e.g., Martin/Hopkins, Sampson equation) provide better VLDL estimation across the lipid spectrum.

How to Lower VLDL

  • Reduce simple sugars and refined carbohydrates
  • Lose excess body fat (≥5–10% weight loss is highly effective)
  • Limit alcohol intake
  • Increase omega-3 fatty acids (EPA/DHA 2–4 g/day)
  • Aerobic exercise ≥150 min/week
