Points from 5 ultrasound features determine the TI-RADS level (TR1–TR5). Higher levels indicate greater malignancy risk. FNA thresholds decrease as suspicion increases. TR1–TR2: benign, no intervention. TR3: FNA ≥ 2.5 cm. TR4: FNA ≥ 1.5 cm. TR5: FNA ≥ 1 cm.
