Heart rate / systolic BP – a quick hemodynamic marker.
SI = HR / SBP. Normal 0.5–0.7. SI > 0.9 suggests occult shock even with normal vitals individually. SI > 1.0 associated with increased mortality in trauma, sepsis, and PE. It detects hemodynamic instability earlier than HR or BP alone.
