Equianalgesic conversion between opioid medications.
Equianalgesic tables provide approximate conversion ratios between opioids. A 25–50% dose reduction is recommended when rotating to a new opioid due to incomplete cross-tolerance. Ratios: morphine 30mg = oxycodone 20mg = hydromorphone 7.5mg = codeine 200mg. Individual variation is significant; titrate to effect.
